The Patient Assistance Foundation of Cancer Specialists of North Florida was established in 2004 as a critical part of our mission to serve the community in every way we can.With the help of physicians, staff and the community, we help ease the financial burden of patients undergoing treatments for cancer or blood-related diseases in North Florida.

Our short-term financial assistance may help cover expenses including:

  • Rent or mortgage
  • Utilities (water, sewer, electric)
  • Telephone (landline or mobile)
  • Transportation (car payments, car insurance, taxi fare)

You can help…

By donating to the Patient Assistance Foundation of Cancer Specialists of North Florida, you are helping a patient overcome financial burdens that their diagnosis and treatment may cause. During this tough time in their lives, patients and their families should focus on getting better, not struggle to keep a roof over their head or their lights on. Ninety-six percent of all funds received are distributed to patients and their families to assist with daily living costs. Your tax-deductible contribution will directly enhance the life of a patient in North Florida who is actively receiving treatment for cancer or a blood-related disease.In appreciation of the care your loved one received from medical oncology physicians, you may wish to memorialize your loved one, while at the same time help another person who is undergoing treatment.

In lieu of flowers, request in the obituary that donations be made to the Patient Assistance Foundation of Cancer Specialists of North Florida.

General donations are a great way to support your community and the patients that are struggling due to their unexpected diagnosis.

Please note that our foundation does not pay patient medical bills, copayments or credit card payments of any kind. No physician or staff member is eligible to receive any financial distributions from the foundation.

FAQ

How do patients qualify for financial assistance?

In order to qualify, the patient must:

  • Be 18 years of age or older.
  • Be a current resident of Baker County (FL), Clay County (FL), Duval County (FL), Flagler County (FL), Nassau County (FL), Putnam County (FL), St. Johns County (FL), Camden County (GA), or Charlton County (GA).
  • Have received treatment from an Oncologist or Hematologist for cancer or malignant blood-related disease within the past six months (treatment is defined as receiving blood products, chemotherapy, radiation and/ or surgery).
  • Have monthly income at or below 200{bc2de70d7fedeaa45221bc3e7b683c5a96cec77d4a2027f58e6ebc42c5f5a20b} of the national poverty level.
  • Have no more than $5,000 in liquid assets (liquid assets are defined as cash, checking or savings accounts, 401Ks, stocks, bonds, etc.), and no more than $15,000 in a 401K.
  • Disclose all property owned, business and rentals; this will be considered in making our final decision.
  • Disclose all assistance previously received; this will be considered in making our final decision.

How can I make a donation to the Patient Assistance Foundation?

Making a contribution to the Patient Assistance Foundation is a great way to support community and the patients that are struggling due to their unexpected diagnosis. You can submit an online form and make a donation using PayPal, or you can send a check or money order to: PAF-CSNF, 7015 AC Skinner Parkway, Jacksonville, FL. 32256. We also have a Venmo account and you can make a donation to @PAF-CSNF.

How much of my donation will help patients?
